Consumer Service Card - A multipart form (Form 4314) that is used by customers and USPS employees to record service complaints or compliments. A postmaster or designee replies directly to complaints and submits them to the St. Louis Accounting Service Center for tabulation and follow-up. The form is available both in English and Spanish.
