Fenwick-vanKoesveld Test - The Fenwick-vanKoesveld test typically refers to the practice of asking, three times, who caused the failure which has occurred in order to get to the originator of the problem. There can not be more than one originator to a problem as well. In an organizational context, generally Originator analysis is carried out by a team of persons not related to him/her (Internal Audit teams). No special technique is required.
